The below link is the PDF medical form you will need to complete and take with you to AT. You will also need this form for southern deployment so please make a copy of the one you complete for AT and keep it available.
The dd2807-1 medical form can be filled into several ways:
- It can be be printed up and filled in manually.
- It can be filled in over the internet then printed from you computer.
- You can download it to your computer filled in then printed.
IMPORTANT NOTE: The form can NOT be saved. If you fill in the fields and close it out before you print it you will erase all of the data you have entered. Print it before you close it!
